Try the new NHS App

If you’re a patient at our practice you can now use the new NHS App, a simple and secure way to access a range of NHS services on your smartphone or tablet. You can use the NHS App to check your symptoms and get instant advice, book appointments, order repeat prescriptions, view your GP medical record and more.

If you already use Patient access, Evergreen, Life, Dimec or My GP app you can continue to use it. You can use the NHS App as well. For more information go to www.nhs.uk/nhsapp 

There are now a large range of health related apps available. Check your app store for options but please ask us if you need advice.

New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Please note when looking at the map the inner boundary (pink shaded area) represents the residential area within which we will accept new patients onto our list.

The outer boundary (orange shaded area) is an extended area and existing patients moving home can remain on our list, if the new address is inside this area.

Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require one form of photographic ID.

You will need to complete a registration form (GMS1 - see below) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.

You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
